Instructions

  1. Print on 8.5 x 11,uncoated,20lb,white paper.

  2. Print page 1 on front. Print page 2 on back.

  3. Page 1 printer settings: Portrait, fit

  4. Page 2 printer settings: Landscape, fit

  5. Fill out and sign the circulator information at the bottom of the 10 line sheet.

  6. When completed, send to PO Box 449, Roseburg, OR, 97470
